Consent to Collect and Use Medical and Personal Information

By providing your information to TLC Nutrition, you give permission for TLC Nutrition to collect and use personal and medical information in order to provide quality nutrition services.

TLC Nutrition collects personal information to help provide professional services to clients. Your information may be used to contact you and may be shared with your medical practitioner or other health care providers when necessary for your care.

This information also helps provide the administration details required to create a client account and assist with billing and rebates through Medicare.

Use of Personal Information

By using TLC Nutrition services, you acknowledge and understand the following:

  • The reasons why your information is being collected.

  • You have the right to access the information you have provided, except in certain circumstances where an explanation will be provided if access is restricted.

  • You may choose not to provide requested information; however, failing to provide certain information may affect the quality of care provided.

  • If the information is required for purposes other than those stated, further consent will be requested.

  • You have read and agreed to the terms and conditions of TLC Nutrition.

  • You agree to receive email communication from TLC Nutrition.

Legal Jurisdiction

This agreement and the terms contained within the use of TLC Nutrition services are governed by and constructed in accordance with the laws of the State of Victoria, Australia. Any disputes will fall under the exclusive jurisdiction of the courts of Victoria, Australia.

Appointment Cancellation Policy

A minimum of 24 hours notice (business days) is required for appointment cancellations to prevent a late cancellation fee.

Any costs associated with recovering late cancellation fees will be passed on to the client.

Consent

By using the services of TLC Nutrition, you consent to the handling of your information for the purposes outlined above, subject to any limitations on access or disclosure that you notify the business of.
